How do you play Truth or Dare with two people?

Take turns offering the choice. One of you asks "truth or dare", the other picks, and you read out whichever comes next. With two people it is worth agreeing up front that anyone can pass once per round without a forfeit.

What are good truth questions for couples?

Specific ones. "What is the smallest thing that has ever started a real argument between us" gets you somewhere; "do you love me" does not. The best ones are answerable in a sentence and still slightly uncomfortable.
